Job Summary
This position is responsible for intake and case management of clients, including but not limited to checking and verifying identification to determine if client qualifies for services. This individual provides case management services to clients including completing client assessments to develop a case management plan, locating resources that promote clients in moving towards self-sufficiency. The Case Manager works closely with the Executive Director to inform and collaborate regarding client case management plans.
